Weight & Metabolic Health

WHAT IS Weight & Metabolic Health ?

Weight and metabolic health are influenced by many interconnected factors. These may include genetics, hormones, medical conditions, medicines, sleep, stress, mental health, disability, life stage, nutrition, movement, culture, finances and access to suitable healthcare.

At Pop’In Health & Recovery, we take a respectful, person-centred and non-judgemental approach. We do not assume that weight is simply a matter of willpower, and we do not use shame, pressure or unrealistic promises.

Our nurse practitioners will work with you to understand your health, identify relevant risks and develop a practical care plan based on your circumstances, preferences and goals.

What Is a Weight & Metabolic Health Consultation?

A Weight & Metabolic Health Consultation is a comprehensive nurse practitioner assessment focused on the factors affecting your weight, metabolism, physical health and overall wellbeing.

The consultation is not limited to weight loss. It may also focus on:

  • Improving metabolic health

  • Reducing preventable health risks

  • Understanding unexplained weight changes

  • Improving energy and daily functioning

  • Supporting healthier and more sustainable routines

  • Reviewing possible medical contributors

  • Monitoring blood pressure, blood glucose or cholesterol

  • Supporting sleep, movement and nutrition

  • Addressing the emotional impact of weight concerns

  • Coordinating care with other health professionals

You are welcome to discuss weight concerns during a General Health Consultation. However, a dedicated Weight & Metabolic Health Consultation allows additional time for a more detailed history, assessment and personalised care plan.

Who May Benefit From This Consultation?

A Weight & Metabolic Health Consultation may be appropriate when you:

  • Are concerned about weight gain or weight loss

  • Have experienced unexplained changes in weight

  • Have found it difficult to maintain previous changes

  • Are concerned about your metabolic health

  • Have a family history of diabetes or cardiovascular disease

  • Have been advised that your blood pressure, glucose or cholesterol requires attention

  • Experience low energy, poor sleep or reduced mobility

  • Are concerned about possible hormonal or medical contributors

  • Have experienced weight changes after starting or changing treatment

  • Want a health-focused assessment before changing your diet or activity

  • Have struggled with repeated restrictive diets or short-term programs

  • Want realistic and sustainable goals

  • Need support coordinating several health professionals

  • Want to improve your health without focusing only on the number on the scales

You do not need to have a particular body size or diagnosis to request an assessment.

Weight and Health Are Complex

Body weight can be affected by more than food intake and physical activity.

Our nurse practitioners may explore factors such as:

  • Genetics and family history

  • Metabolism

  • Hormonal and endocrine health

  • Current and previous medical conditions

  • Current and previous treatments

  • Medicines and supplements

  • Sleep quality and possible sleep disorders

  • Stress and mental wellbeing

  • Menstrual, reproductive or menopause-related changes

  • Mobility, pain or disability

  • Neurodivergence and sensory needs

  • Eating patterns and access to food

  • Alcohol, tobacco and other substance use

  • Shift work and irregular routines

  • Housing, finances and transport

  • Culture, family and community

  • Previous experiences with healthcare

  • Weight stigma or discrimination

Our aim is to understand the whole person rather than making assumptions based on appearance, body weight or a single measurement.

What Happens During the Consultation?

Our nurse practitioners will begin by discussing your main concerns, health goals, weight history and any changes you have noticed.

They may ask about:

  • Changes in weight over time

  • Previous attempts to improve weight or metabolic health

  • Current eating patterns

  • Physical activity and mobility

  • Sleep and fatigue

  • Stress and emotional wellbeing

  • Current medical conditions

  • Current medicines and supplements

  • Family health history

  • Menstrual, hormonal or reproductive health

  • Digestive symptoms

  • Pain or physical limitations

  • Daily routines

  • Alcohol, smoking or other substance use

  • Previous blood tests or investigations

  • What has and has not worked for you before

  • Your preferences and concerns about treatment

The consultation is intended to be collaborative. You will have the opportunity to explain what matters to you, ask questions and discuss the type of support you feel ready to consider.

Clinical Assessment

Depending on your needs and whether the appointment is conducted by telehealth or face-to-face, the assessment may include:

  • Review of your medical and mental-health history

  • Review of current and previous treatments

  • Review of medicines and supplements

  • Weight history

  • Height and weight measurements where appropriate

  • Body mass index as one possible screening measure

  • Waist measurement where appropriate

  • Blood pressure

  • Relevant physical observations

  • Review of previous pathology or imaging

  • Screening for metabolic risk factors

  • Screening for sleep concerns

  • Screening for mental-health or eating-related concerns

  • Assessment of how your health affects everyday functioning

  • Consideration of your personal and family health risks

Measurements such as body mass index and waist circumference are guides only. They are not the sole basis for assessing your health, and they may not be appropriate or accurate for every person.

Health Concerns We May Explore

A Weight & Metabolic Health Consultation may include assessment of concerns associated with:

  • Weight gain or unexplained weight change

  • Elevated blood pressure

  • Blood glucose concerns

  • Cholesterol or lipid concerns

  • Insulin resistance or metabolic risk

  • Fatigue or reduced energy

  • Sleep difficulties

  • Possible sleep apnoea symptoms

  • Reduced mobility

  • Joint discomfort

  • Hormonal or menstrual changes

  • Menopause-related changes

  • Digestive concerns

  • Medication-related weight changes

  • Emotional eating or disrupted routines

  • Stress, anxiety or low mood

  • Reduced confidence or avoidance of healthcare

  • Family history of metabolic or cardiovascular conditions

Not every concern can be fully assessed during one appointment. Further information, investigations or follow-up may be required.

Investigations and Health Monitoring

Where clinically appropriate and eligible, our nurse practitioners may request or review relevant investigations.

These may help assess:

  • Blood glucose regulation

  • Cholesterol and other lipids

  • Liver health

  • Kidney health

  • Thyroid or other endocrine concerns

  • Nutritional status

  • Cardiovascular risk factors

  • Possible contributors to fatigue or weight change

  • Monitoring requirements associated with existing treatment

The investigations requested will depend on your symptoms, history, previous results and clinical assessment.

Our nurse practitioners are responsible for reviewing investigations they initiate and arranging appropriate follow-up.

Developing Your Care Plan

Following the assessment, our nurse practitioners will discuss their findings with you and work with you to develop an individual care plan.

Your plan may include:

  • Health education

  • Realistic and measurable goals

  • Changes to everyday routines

  • Nutrition-related strategies

  • Movement or physical-activity planning

  • Sleep support

  • Stress-management strategies

  • Monitoring symptoms or health measurements

  • Treatment of relevant health conditions

  • Review of current medicines

  • Further investigations

  • Referral to another health professional

  • Follow-up consultations

  • Coordination with your existing healthcare team

Recommendations will be based on your individual health needs, preferences, readiness for change and available evidence.

There is no single plan that is suitable for everyone.

Focusing on Health, Function and Sustainable Change

Success does not need to be measured only by kilograms lost.

Depending on your goals, progress may also include:

  • Improved energy

  • Better sleep

  • Improved mobility or physical comfort

  • Improved blood pressure

  • Improved blood glucose or cholesterol

  • More regular eating patterns

  • Greater confidence preparing meals

  • Increased participation in meaningful activities

  • More consistent healthcare

  • Reduced distress about food or weight

  • Better understanding of your health

  • Improved daily routines

  • Reduced risk of preventable health complications

  • Feeling more confident making informed health decisions

The focus is on changes that are safe, realistic and sustainable rather than rapid or extreme results.

Working With Other Health Professionals

Weight and metabolic health may require support from more than one practitioner.

With your consent, our nurse practitioners may communicate or coordinate care with:

  • Your GP or general practice

  • Accredited practising dietitians

  • Exercise physiologists

  • Physiotherapists

  • Psychologists or counsellors

  • Pharmacists

  • Endocrinologists

  • Cardiologists

  • Sleep-health services

  • Gastroenterology services

  • Women’s or men’s health specialists

  • Mental-health practitioners

  • Other relevant health or allied-health services

Where another practitioner is better placed to provide part of your care, we will explain why and discuss an appropriate referral pathway.

We aim to work with your existing healthcare providers rather than unnecessarily duplicate or replace their care.

Your Regular GP Remains Important

We strongly encourage patients to maintain a relationship with a regular GP or general practice.

A regular practice can support:

  • Continuity of long-term healthcare

  • Access to your broader medical history

  • Management of complex or ongoing conditions

  • Preventive health and screening

  • Care during periods when our service is unavailable

  • Coordination with specialists and hospitals

  • Management of health concerns outside our service scope

With your consent, relevant consultation summaries, investigation results, referrals and care recommendations may be communicated to your nominated GP or treating practitioner.

People without a regular GP may still request an appointment. However, our nurse practitioners may recommend establishing care with a general practice where ongoing or complex medical management is required.

Connected Clinical and Recovery Support

Knowing what may improve your health does not always make the changes easy to implement.

Some people benefit from practical support to:

  • Establish routines

  • Prepare for appointments

  • Organise health information

  • Attend follow-up appointments

  • Understand agreed recommendations

  • Plan shopping or meal-preparation routines

  • Build confidence using community services

  • Develop reminders and tracking systems

  • Communicate with health providers

  • Address disability-related barriers

  • Work toward practical health and recovery goals

With your informed consent, our nurse practitioners may coordinate relevant aspects of care with the Pop’In Health & Recovery recovery and capacity-building service stream.

Recovery, Support Coordination and Capacity Building services remain separate from clinical care. They do not diagnose health conditions, prescribe treatment or change clinical instructions.

You can access a Weight & Metabolic Health Consultation without using any recovery, coaching or NDIS service.

Mental Health, Eating Patterns and Body Image

Weight concerns can affect emotional wellbeing, confidence, relationships and willingness to access healthcare.

Mental-health concerns may also affect:

  • Appetite

  • Motivation

  • Sleep

  • Energy

  • Shopping and meal preparation

  • Physical activity

  • Routines

  • Impulse control

  • Emotional eating

  • Confidence attending appointments

Our approach is intended to be respectful and free from shame.

Where there are concerns about:

  • Disordered eating

  • Binge eating

  • Restrictive eating

  • Purging

  • Significant body-image distress

  • Rapid or unsafe weight change

  • Anxiety around food

  • Depression or self-harm risk

our nurse practitioners may recommend additional assessment or referral to a suitable mental-health, dietetic or specialist eating-disorder service.

A Weight & Metabolic Health Consultation is not a replacement for specialist eating-disorder treatment.

Reviewing Medicines and Supplements

Some medicines, health conditions and supplements may affect appetite, weight, energy, sleep or metabolic health.

Our nurse practitioners may review:

  • Current prescribed medicines

  • Over-the-counter products

  • Vitamins and supplements

  • Previous treatment

  • Side effects

  • Interactions

  • Monitoring requirements

  • Whether another prescriber should be contacted

  • Whether further review is required

Do not stop or change prescribed treatment without advice from an appropriately qualified health practitioner.

A consultation does not guarantee that any medicine will be started, continued, changed or stopped.

Treatment Decisions

Treatment decisions are made after an individual clinical assessment.

The appropriate approach may include:

  • Education and monitoring

  • Lifestyle and routine-based strategies

  • Management of contributing health conditions

  • Referral to another practitioner

  • Further investigation

  • Review of existing treatment

  • Ongoing clinical follow-up

  • Other clinically appropriate options within the nurse practitioner’s authorised scope

Booking an appointment does not guarantee a prescription or any particular treatment.

The consultation fee covers the practitioner’s time, assessment, professional advice and care—not a predetermined outcome.

Telehealth and Face-to-Face Consultations

  • Reviewing your health history

  • Discussing weight or metabolic-health concerns

  • Reviewing previous results

  • Discussing sleep, stress and routines

  • Physical examination

  • Accurate clinical measurements

  • Blood pressure or other observations

  • Significant or rapidly changing symptoms

  • Concerns that cannot be adequately assessed by telehealth

  • Situations where examination equipment is required

  • Concerns requiring urgent medical assessment

Our nurse practitioners will advise when an in-person consultation, GP review, urgent-care service or emergency department is more appropriate.

Frequently Asked Questions

Is this just a weight-loss appointment?

No. The consultation focuses on your broader health, including metabolic risk, physical health, mental wellbeing, sleep, medicines, lifestyle, function and personal goals.

Weight change may be one goal, but it is not the only way progress can be measured.

Will I be judged because of my weight?

Our service is intended to be respectful, person-centred and non-judgemental.

We recognise that body weight is affected by many biological, medical, psychological, social and environmental factors.

Do I need to reach a particular weight to book?

No. You can request a consultation at any body size if you have concerns about weight, metabolic health, unexplained changes or related health risks.

Do I need a GP referral?

A GP referral is not required for most nurse practitioner consultations.

However, we strongly encourage you to maintain a relationship with a regular GP or general practice for continuity of care.

Can I discuss several health concerns?

Yes. Weight and metabolic health often overlap with sleep, mental health, medicines, hormonal concerns and other medical conditions.

The available appointment time may limit how many issues can be safely assessed. A follow-up or another consultation type may be recommended.

Will my weight be measured?

Measurements may be recommended where they are clinically useful and appropriate.

Your consent, comfort and individual circumstances will be considered. Weight is not the only measure used to assess your health.

Is body mass index the only measure you use?

No. Body mass index may be used as one screening tool, but it has limitations and is not appropriate for every person.

Our nurse practitioners consider your broader health, medical history, body composition, waist measurement where appropriate, pathology, functioning, risks and goals.

Can a nurse practitioner request blood tests?

Where clinically appropriate and eligible, our nurse practitioners may request relevant pathology or other diagnostic investigations.

They are also responsible for reviewing the results they initiate and arranging appropriate follow-up.

Can a nurse practitioner diagnose metabolic conditions?

Our endorsed nurse practitioners may conduct assessments, apply diagnostic reasoning and formulate diagnoses within their professional scope.

Further investigation or referral may be required before a diagnosis can be confirmed.

Will I receive a prescription?

Not necessarily.

Any prescribing decision depends on your clinical assessment, health history, current treatment, risks, monitoring needs and the practitioner’s authorised scope. No particular treatment is guaranteed.

Can you review treatment prescribed elsewhere?

Our nurse practitioners can review your health and current treatment. Relevant records or communication with your existing prescriber may be required.

Continuation or alteration of treatment is not automatic.

Do you provide meal plans?

Our nurse practitioners can provide general health education and discuss nutrition-related goals.

A detailed or therapeutic meal plan may require referral to an accredited practising dietitian.

Can you help if my medicines have affected my weight?

Yes. Current and previous medicines can be reviewed as part of the broader assessment.

Do not stop or change prescribed treatment without appropriate clinical advice.

Can you help with weight concerns linked to mental health?

Yes. Mental health, stress, sleep, motivation and eating patterns can be discussed during the consultation.

Additional counselling, psychology, dietetic or specialist support may be recommended.

Can I access this service by telehealth?

Yes. Many parts of the assessment and follow-up can be provided by telehealth.

An in-person assessment may be required where physical examination, measurements or observations are necessary.

Can you refer me to other health professionals?

Where appropriate and eligible, our nurse practitioners may refer or recommend that you consult another suitable health, allied-health or specialist service.

Can you guarantee that I will lose weight?

No. No particular amount or rate of weight change can be guaranteed.

The service focuses on safe assessment, informed decision-making and realistic long-term health goals.

Can I access recovery or capacity-building support as well?

Yes. Practical recovery or capacity-building support may help with routines, appointments, organisation and implementing agreed strategies.

These services are separate from clinical care and are not compulsory.

What happens if this service is not appropriate for me?

Our nurse practitioners will explain the limitation and, where possible, recommend a suitable GP, dietitian, psychologist, specialist, urgent-care or emergency pathway.
